Full name, teacher's name, course/class name (or period, either works for Connolly), and date, each on a new line. Located at the top left corner with an inch margin
How to write the date

Header information and location
Last name and the page number, located at the top right corner with a � inch margin (ex: Maxwell-Sommer 2)
Title formatting
Should be one line down from the heading, centered, and capitalized accordingly

Evenly double spaced throughout
What information is cited in the paper
All information that is not common knowledge (ex: "the sky is blue" would not be cited, but "the sky is blue because of light refracting off of particles blah blah blah" would require citation)
