Needle and Syringe SELECTION GUIDE

ID

needle gauge: 25-27
needle length: inch to 5/8 inch
syringe: TB

syringe selection for IM or SQ injection of less than 1 mL need hundredths of a mL
IM/SQ of less than 0.2mL

syringe: TB

Needle/syringe selection for insulin

only use insulin syringes (SQ)
(gauge: 27-32,
length: 3/8 inch to inch)

Needle/syringe selection for intramuscular injection for an adult in deltoid

needle gauge: 22-24,
length: 1 inch

Needle/syringe selection for intramuscular injection for an adult in the ventrogluteal

needle gauge: 22-24,
length: 1 inch
Greater trochanter landmark

Needle/syringe selection for intramuscular injection of a viscous or oil-based solution

gauge: 20-21

Needle selection for overweight/obese intramuscular in deltoid

needle gauge: 22-24,
length: 1 inch

Needle selection for obese intramuscular in ventrogluteal

needle gauge: 22-24,
length: 2 inches

Needle selection for morbidly obese intramuscular in ventrogluteal

needle gauge: 22-24,
length: 2 inches

Tuberculin syringe is used for

1 ml syringe,
used for doses less than 1 ml;
allows for calibration precision to the hundredths place

3 ml syringe

used for doses greater than 1 ml
or for doses that require calibration to the tenths place only
Most routine injections of 0.2-3mL

where do you do an intradermal injection and with what needle/gauge size?

injection placed just below the epidermis on lower arm same side as palm
Usually used for skin testing for TB or other diseases (allergy testing)
Use a small needle, with a 15-degree angle of insertion (25-27 gauge and to 5/8 inch)
Use a 1 mL tuberculin

Intramuscular Route: Z-Track Method (p.696)

Reduces pain caused by irritating drugs leaking into subcutaneous tissue such as iron dextran or Vistaril
Seals the medication in the muscular layer
With non-dominant hand, retract the skin laterally
Cleanse the site
Insert needle at 90 degree angle
Maint

most routine injections of 0.2 mL to 3 mL

3 mL syringe

IM or SQ injections less than 0.2 mL

1 mL TB syringe

intradermal injection

1 mL syringe

mixing and transfer

5 mL to 20 mL syringe depending on volume of diluent to be added or volume to be transferred
